Job summary

1st Choice, LLC is seeking RNs to perform UAS NY assessments through in-home visits and telehealth in New York. You will apply clinical judgment to determine care needs and document findings that support services for home-based clients.

The role offers an Independent schedule with hybrid in-person and telehealth assessments, mileage reimbursement, and ongoing clinical support. An RN license in NY and BSN are required.

Qualifications

• Must have 1-2 years of RN experience in one-on-one in-home or community care.
• RN license valid in New York state.
• BSN is required for this role.
• Familiarity with UAS-NY is a plus.

Responsibilities

• Complete UAS NY assessments during in-home visits or telehealth.
• Review medical documentation and record accurate findings.
• Educate consumers and communicate with providers when needed.
• Maintain timely, high-quality documentation meeting NYS standards.
• Manage schedule, route planning and daily workflow.
• Follow conflict-free requirements under NYS DOH.
