Personal Touch Home Aides of New York is seeking a qualified UAS Assessment Nurse (RN) to perform in-person UAS assessments and document results for eligibility and care planning in clients' homes across New York. The role requires NY RN licensure and UAS-NY certification with experience in home care or related fields.

Responsibilities include accurate documentation, collaboration with care teams, and adherence to timelines.

Qualifications

• Registered Nurse licensed in New York State (RN).
• UAS-NY certification is required.
• Experience in home care, case management, or long-term home care is preferred.
• Proficiency with the DOH UAS assessment instrument.

Responsibilities

• Conduct UAS assessments to determine level of care and support needs.
• Document assessment results using the Uniform Assessment System (UAS) tool.
• Provide compassionate interactions with clients in community settings.
• Submit assessments within designated timeframes and maintain records.
• Collaborate with other care providers for comprehensive assessment.
• Attend trainings and stay updated on assessment changes.
