Qualifications

• Must have a valid NY RN license and UAS-NY certification.
• Experience in home care, case management or long-term home care.
• Proficient with DOH UAS assessment instrument.
• Bachelor’s degree in nursing preferred.

Responsibilities

• Conduct UAS assessments for clients to determine level of care and support needs.
• Utilize the Uniform Assessment System (UAS) tool to document assessment results.
• Provide compassionate, professional interactions with clients in community settings.
• Submit completed assessments within designated timeframes and maintain records.
• Collaborate with other care providers to ensure a comprehensive assessment.
• Attend required training sessions and stay informed of assessment updates.
