Job summary

1st Choice, LLC is seeking a Registered Nurse UAS Assessor to join our NY team. This hybrid role involves a mix of in-home and telehealth UAS NY assessments, with emphasis on clinical judgment and independent scheduling.

The ideal candidate will hold an NY RN license, a BSN, and have 1–2 years of in-home or community-based care experience. You will navigate NYS DOH requirements and maintain thorough documentation while traveling between assessment locations.

Qualifications

• RN license in New York is required.
• BSN required.
• Strong communication and time management are essential.
• Proficient in Electronic Health Records (EHR).

Responsibilities

• Complete UAS NY assessments during in-home visits or via telehealth.
• Review medical documentation and record accurate, detailed findings.
• Educate consumers and communicate with providers when needed.
• Maintain timely, high-quality documentation that meets NYS standards.
• Manage schedule, route planning, and daily workflow.
• Follow all conflict-free requirements under NYS DOH.
