As a UAS Assessment RN, you will conduct thorough in-home evaluations using NY UAS-NY to determine eligibility for home care services and appropriate care planning, while coordinating with interdisciplinary teams to ensure high-quality patient outcomes.

Qualifications

• Active New York State RN license is required.
• Current UAS-NY Certification preferred or willingness to obtain.
• Minimum 1 year nursing experience.
• Home health/MLTC/case management/UAS experience preferred.
• Strong clinical assessment, critical thinking, and decision-making skills.
• Excellent communication and documentation abilities.
• Ability to work independently and manage appointments and documentation.

Responsibilities

• Conduct comprehensive UAS-NY assessments in patients' homes across Suffolk County.
• Evaluate medical, functional, cognitive, psychosocial, and environmental needs.
• Complete accurate documentation in UAS-NY and EMR systems.
• Develop objective assessments to support care planning and service recommendations.
• Educate patients and families about assessment processes and available home care services.
• Collaborate with care teams to ensure continuity of care and compliance with regulations.
• Travel throughout assigned service areas with a flexible schedule.
