Job summary

Creative Solutions In HealthCare is seeking a Per Diem Speech Language Pathologist (SLP) to join our in-house team serving residents with person-centered care.

The role focuses on evaluating swallowing and cognitive deficits, developing individualized treatment plans, and implementing AAC systems while educating residents and caregivers. This position offers flexible schedules within a SNF/long-term care setting in Texas.

Qualifications

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Develop and implement individualized treatment plans to increase client’s independence.
• Provide AAC systems for individuals with severe expressive and/or language comprehension disorders.
• Educate residents and caregivers.
• Coordinate and collaborate with other professionals for diagnostic swallowing procedures as ordered by physician.
• Demonstrate competent clinical skills according to current Speech Therapy standards of practice.
• Communicate changes in resident’s condition.
• Provide accurate and timely billing records and supporting clinical documentation.
