Job summary

Creative Solutions In HealthCare is seeking a Per Diem (PRN) Speech-Language Pathologist to join our in-house rehab team serving residents in a Texas facility. The role focuses on evaluating swallowing and cognitive deficits, and delivering individualized therapy plans to improve independence and function.

Responsibilities include providing AAC, educating residents and caregivers, coordinating with physicians and other professionals, and ensuring accurate billing and documentation.

Qualifications

• Texas license in Speech-Language Pathology is required.
• ASHA certification is required.
• Experience in SNF/long-term care preferred.
• Knowledge of Net Health EDS is preferred.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Develop and implement individualized treatment plans to improve swallowing and cognition.
• Provide AAC systems for expressive/language disorders.
• Educate residents, caregivers, and professionals.
• Coordinate with other professionals for diagnostic procedures.
• Maintain accurate billing and clinical documentation.
• Ensure confidentiality and comply with laws.
