Job summary

Creative Solutions in Healthcare, Inc. is seeking a Per Diem Speech Language Pathologist to join our in-house rehab team serving residents in a SNF setting. The role focuses on swallowing and cognitive therapy, AAC implementation, and collaborative care.

Texas licensure and ASHA certification are required; new grads are welcome. Flexible schedules and competitive compensation are offered for a PRN position based in Texas.

Qualifications

• SNF/long-term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Assist residents to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled, medically necessary SLP assessments and develop individualized treatment plans.
• Provide AAC systems for individuals with expressive/language disorders.
• Coordinate and collaborate with other professionals for diagnostic swallowing procedures as ordered by physician.
• Maintain accurate billing records and clinical documentation.
• Coordinate training with restorative nursing and other staff.
