Job summary

Creative Solutions in Healthcare, Inc. seeks a Per Diem Speech-Language Pathologist (SLP) to join our in-house team serving residents in a SNF setting. You will assess swallowing and cognition, develop treatment plans, and implement therapies to maximize independence.

In-house rehab and flexible scheduling are offered. Active Texas SLP license and ASHA certification required; experience in long-term care is preferred, with familiarity in Net Health EHR advantageous.

Qualifications

• Experience in SNF/long-term care settings is preferred.
• Knowledge of current speech-language pathology treatment practices.
• Familiarity with Net Health electronic documentation system helpful.
• Ability to work with residents of varying personalities.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Develop and implement individualized treatment plans.
• Provide AAC systems for expressive/language disorders.
• Coordinate with other professionals for diagnostic procedures when needed.
• Maintain accurate billing records and clinical documentation.
