Job summary

UnitedHealth Group is seeking a Per Diem Senior Living Physical Therapist to evaluate and treat adult to geriatric patients in outpatient and home settings. The role includes developing care plans, supervising PTA/tech, and participating in team conferences.

Requires current PT licensure in the practicing state and a valid driver's license. CPR certification is preferred. Compensation includes per-visit and hourly pay with comprehensive benefits and 401k eligibility.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient to determine rehab potential and creates an individualized plan of care.
• Sets initial STG's and LTG's and documents progress.
• Utilizes safe treatment techniques and maintains equipment logs.
• Completes discharge summaries on final visits.
• Provides care suitable for adult to geriatric patients; participates in team conferences.
