Job summary

LHC Group is seeking a Per Diem Senior Living Physical Therapist to deliver age-appropriate services for adult to geriatric patients in outpatient settings. Responsibilities include assessments, direct treatment, supervising PTA/tech, home exercise programs, and documentation according to facility standards.

Required licensure and a driver’s license with reliable transportation are essential. Strong emphasis on safety, teamwork, and patient/family education drives patient outcomes.

Qualifications

• Active Physical Therapy license in the state of practice.
• Current driver's license with reliable transportation.

Responsibilities

• Evaluate and treat patients age-appropriate; develop plan of care.
• Supervise PTA or tech delivering therapy.
• Develop home exercise program and provide patient/family instruction.
• Participate in team and family conferences; document services.
• Select adaptive equipment and participate in PI programs.
• Complete discharge summaries for outpatient PT.
• Provide safe care regardless of patient background.
