Job summary

LHC Group in Sarasota, FL, part of UnitedHealth Group's Optum family, is seeking a Per Diem Senior Living Physical Therapist to deliver age-appropriate care across adult and geriatric populations.

You will assess, treat, document, and collaborate with the PTA/tech and care team, developing home exercise programs and participating in PI programs while adhering to facility standards. This role emphasizes direct patient care and flexible scheduling.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient to determine rehab potential and sets an individualized plan of care with goals for the team plan.
• Sets initial STG's and LTG's.
• Utilizes equipment in a safe and proper manner; maintains equipment logs and reports faults.
• Maintains equipment logs for required equipment in the outpatient clinic.
• Provides direct patient care and develops a home exercise program; documents services per department standards.
