Job summary

Karing Hearts Home Health in Ventura, CA is seeking a compassionate Per Diem RN to join our Home Infusion team. You will deliver IV therapies in patients' homes, perform assessments, and educate patients and caregivers on infusion therapy and home care protocols.

The ideal candidate holds a California RN license in good standing, with 1–2 years of IV infusion or home health experience, proficiency with PICC/port access, and the ability to work independently in a home setting.

Qualifications

• Active California RN license in good standing.
• Minimum 1–2 years of IV infusion or home health nursing experience preferred.
• Proficiency in peripheral and central line access, including PICC and port management.
• Strong clinical assessment and critical thinking skills.
• Excellent communication skills and ability to work independently in a home setting.
• Valid California driver's license and reliable transportation required.
• CPR/BLS certification required.

Responsibilities

• Administer IV infusion therapies including antibiotics, TPN, chemotherapy, and hydration in patient homes
• Perform thorough patient assessments and monitor responses to infusion treatments
• Educate patients and caregivers on infusion therapy, medication management, and home care protocols
• Maintain, troubleshoot, and manage IV access devices including PICCs, ports, and central lines
• Accurately document patient care, clinical findings, and treatment plans in compliance with regulatory standards
• Collaborate with physicians, pharmacists, and care team members to coordinate patient treatment plans
• Respond to patient concerns and on-call needs as required per diem schedule
