Job summary

Karing Hearts Home Health is seeking a compassionate Per Diem RN to join our Home Infusion team in Tulare, CA. This role delivers high-quality, patient-centered infusion therapy in patients' homes, offering flexibility for a better work-life balance.

You will administer IV therapies, assess patients, educate families, and collaborate with physicians and the care team to ensure optimal outcomes. Valid CA RN license, CPR/BLS, and access to a vehicle are required.

Qualifications

• Active California RN license in good standing.
• Minimum 1–2 years IV/infusion therapy or home health nursing experience preferred.
• Proficiency in peripheral IV insertion and central line access/management.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Valid California driver's license, reliable transportation, and current auto insurance.
• CPR/BLS certification required.
• Ability to work independently with a flexible per diem schedule.

Responsibilities

• Administer IV infusion therapies in the home setting (antibiotics, TPN, hydration, and specialty meds).
• Perform comprehensive patient assessments and monitor responses to infusion treatments.
• Educate patients and caregivers on infusion therapy, medication management, and safety protocols.
• Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports).
• Collaborate with physicians, pharmacists, and the care team to ensure optimal patient outcomes.
• Accurately document patient care, clinical findings, and treatment progress per regulatory standards.
• Respond to patient concerns and on-call needs per diem.
