Job summary

Karing Hearts Home Health is seeking a compassionate Per Diem RN to join our Home Infusion team in San Diego, CA. This role offers an independent, patient-centered environment with opportunity to deliver high-quality infusion therapy directly to patients in their homes.

Requirements include an active California RN license, 2+ years of RN experience (preferably in home infusion/IV therapy), and BLS/CPR. Knowledge of central venous access devices and EMR documentation is essential.

Qualifications

• Active California RN license in good standing is required.
• Minimum 2 years RN experience, preferably in home infusion or IV therapy.
• CRNI certification preferred and BLS/CPR current.

Responsibilities

• Administer IV infusion therapies in patient homes (antibiotics, TPN, chemotherapy, biologics).
• Perform patient assessments and monitor responses to infusion treatments.
• Insert and manage PICCs, ports and tunneled catheters.
• Educate patients and caregivers on infusion therapy and safety protocols.
• Document care and treatment progress in EMR systems.
• Coordinate with physicians and care team on treatment plans.
• Respond to infusion-related complications in the field.

Responsibilities:

• Administer IV infusion therapies including antibiotics, TPN, chemotherapy, and biologics in patient home settings
• Perform thorough patient assessments and monitor responses to infusion treatments
• Insert and manage peripheral IVs, PICCs, ports, and other central venous access devices
• Educate patients and caregivers on infusion therapy, medication management, and safety protocols
• Accurately document patient care, clinical findings, and treatment progress in EMR systems
• Collaborate with physicians, pharmacists, and care team members to coordinate patient treatment plans
• Respond to and manage infusion-related complications or adverse reactions in the field

Requirements:

• Active California RN license in good standing
• Minimum 2 years of RN experience, preferably in home infusion, infusion clinic, or IV therapy
• Proficiency with central venous access devices including PICCs, ports, and tunneled catheters
• Strong phlebotomy and peripheral IV insertion skills
• CRNI (Certified Registered Nurse Infusion) certification preferred
• Current BLS/CPR certification required
• Reliable transportation and valid California driver's license
• Excellent communication skills, self-motivation, and ability to work independently
