Job summary

Karing Hearts Home Health in East Bakersfield, CA is seeking a compassionate Per Diem RN to join our Home Infusion team. You will deliver high-quality, patient-centered infusion therapy in patients' homes.

This role offers independence, a flexible schedule, and opportunities for advancement while collaborating with physicians and the care team. Requirements include CA RN license, 1–2 years IV/infusion, CPR/BLS, and a valid driver's license for home visits.

Qualifications

• Active California RN license in good standing.
• 1–2 years IV/infusion therapy or home health nursing preferred.
• Proficiency with central venous access devices (PICC lines, ports) and infusion pumps.
• Strong clinical assessment and communication skills.
• Valid California driver's license and reliable transportation for home visits.
• Ability to work independently with minimal supervision in a home care setting.
• CPR/BLS certification required.

Responsibilities

• Administer IV infusion therapies (antibiotics, TPN, chemotherapy, hydration) in home settings.
• Perform patient assessments, monitor vitals, and evaluate response to treatment.
• Educate patients and caregivers on infusion therapy, medications, and safety protocols.
• Insert, maintain, and troubleshoot peripheral/central venous access devices.
• Document care and medication administration in compliance with regulatory standards.
• Collaborate with physicians, pharmacists, and care teams to coordinate plans.
• Respond to patient concerns and on-call needs as required.

Responsibilities:

• Administer IV infusion therapies including antibiotics, TPN, chemotherapy, and hydration in the home setting
• Perform patient assessments, monitor vital signs, and evaluate response to infusion treatments
• Educate patients and caregivers on infusion therapy, medication management, and safety protocols
• Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports, etc.)
• Accurately document patient care, clinical notes, and medication administration in compliance with regulatory standards
• Collaborate with physicians, pharmacists, and care team members to coordinate patient treatment plans
• Respond to patient concerns and on-call needs as required on a per diem basis

Requirements:

• Active California Registered Nurse (RN) license in good standing
• Minimum 1-2 years of IV/infusion therapy or home health nursing experience preferred
• Proficiency with central venous access devices, PICC lines, and infusion pumps
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Valid California driver's license and reliable transportation for home visits
• Ability to work independently with minimal supervision in a home care environment
• CPR/BLS certification required
