Job summary

Karing Hearts Home Health in Riverside, CA is seeking a compassionate Per Diem RN to join our Home Infusion team. Deliver expert infusion therapy in patients' homes, supporting independence and comfort. This role requires a CA RN license, infusion experience, and a driver’s license; CPR/BLS is required.

Qualifications

• Active California RN license in good standing.
• 1–2 years of IV/infusion therapy or home health nursing experience preferred.
• Proficiency in PICC line and central venous catheter care.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, time management, and patient education abilities.
• Valid California driver's license and reliable transportation required.
• CPR/BLS certification required.

Responsibilities

• Administer IV infusion therapies at patients' homes (antibiotics, TPN, chemotherapy, hydration).
• Assess patients and monitor responses to infusion treatments.
• Educate patients and caregivers on infusion therapy and safety.
• Maintain compliant clinical documentation.
• Coordinate care with physicians, pharmacists, and the care team.
• Manage IV access devices including PICCs and central lines.
• Handle, store, and dispose of infusion medications and supplies properly.

Responsibilities:

• Administer IV infusion therapies including antibiotics, TPN, chemotherapy, and hydration in patient homes
• Perform thorough patient assessments and monitor responses to infusion treatments
• Educate patients and caregivers on infusion therapy, medication management, and safety protocols
• Maintain accurate and timely clinical documentation in compliance with regulatory standards
• Coordinate care with physicians, pharmacists, and the interdisciplinary care team
• Manage and troubleshoot IV access devices including PICCs, ports, and central lines
• Ensure proper handling, storage, and disposal of infusion medications and supplies

Requirements:

• Active California RN license in good standing
• Minimum 1-2 years of IV/infusion therapy or home health nursing experience preferred
• Proficiency in PICC line and central venous catheter care
