Job summary

Karing Hearts Home Health in San Bernardino, CA is seeking a compassionate Per Diem RN to join our Home Infusion team. You will deliver high-quality infusion therapies in patients' homes, ensuring comfort, safety, and efficacy, with a focus on personalized patient care.

This flexible, on-demand role offers autonomy, on-call coverage, and opportunities to advance within a supportive home health setting. California RN licensure and IV infusion experience required.

Qualifications

• Active and unrestricted California RN license required.
• Minimum 1-2 years of IV/infusion therapy or home health nursing experience preferred.
• Proficiency in peripheral IV insertion and central line access (PICC, port, tunneled catheters).
• Strong clinical assessment and critical thinking skills.
• Valid California driver's license, reliable transportation, and current auto insurance.
• CPR/BLS certification required.
• Excellent communication skills and ability to work independently in a home care environment.
• Compassionate, patient-centered approach with strong organizational skills.

Responsibilities

• Administer IV infusion therapies including antibiotics, TPN, hydration, and specialty medications in the home setting.
• Perform comprehensive patient assessments and monitor responses to infusion treatments.
• Educate patients and caregivers on infusion therapy, medication management, and safety protocols.
• Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports, central lines).
• Accurately document patient care, clinical findings, and treatment plans in compliance with regulatory standards.
• Collaborate with physicians, pharmacists, and the care team to coordinate and adjust patient treatment plans.
• Respond to patient concerns and on-call needs in a timely and professional manner.
