Good Shepherd Rehabilitation is seeking a per diem Physical Therapist for the Center Valley, PA location. The role supports restoration of function by planning and administering medically prescribed physical therapy, with a day shift across approximately 8 hours, flexible as needed.

Responsibilities include assessing patients, developing and updating care plans, educating patients and families, and coordinating with the healthcare team to ensure quality outcomes.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Previous acute care experience or similar setting required.
• Physical Therapist license required.
• Basic Life Support / CPR required.

Responsabilités

• Assess patient condition upon physician order and at regular intervals during shift.
• Integrate patient information to facilitate holistic patient care.
• Set goals and collaborate with health-care team, patients and families.
• Document interventions and responses and revise care plans as needed.
• Provide education to patients and health-care team related to treatments and continuing care requirements.
• Maintain communication with care management regarding variances from the care plan or transfer/discharge plan.
