Susquehanna Health and Wellness Center is seeking a PRN Physical Therapist (PT) to provide per diem physical therapy services to residents in a large rehab and nursing facility. The role emphasizes compassionate care, adherence to care plans, and collaboration with supervisors for ongoing patient progress.

The ideal candidate will be a graduate of an accredited PT program, hold a valid state license, and have experience in long-term care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Must have a valid professional license by discipline for state.
• Prefer at least one-year experience in LTC.

Responsibilities

• Provide individual therapy services per care plans for residents.
• Assist in resident assessment activities as needed.
• Assist in developing therapy plans with MDS input.
