Job summary

Eligible candidates hold a Bachelor's Degree in Physical Therapy, PA licensure, and BLS; Master's or Doctorate is preferred. Travel to patient care locations is expected.

Qualifications

• Bachelor’s Degree in Physical Therapy required.
• PA PT licensure upon hire.
• BLS certification upon hire.
• Ability to travel to patient care assignments - Home Care.

Responsibilities

• Establishes and implements appropriate physical therapy plans of care to restore function, prevent disability, and help patients reach their maximum performance level.
• Implements an individualized treatment plan using skilled, evidence-based therapy techniques including therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.
• Provides physical therapy treatment aligned with the patient’s needs assessment.
• Performs reassessment when indicated and modifies plans accordingly.
• Communicates patient progress and limitations to supervisor, physician(s), and other team members.
• Provides supervision as required by regulatory requirements.
• Establishes measurable goals and time frames for treatment.
• Provides cost‑effective care in line with billing guidelines.
• Completes and submits documentation per policies and regulatory requirements.
