Qualifications

Responsibilities

• Evaluates, plans, directs, and administers physical therapy programs to restore function.
• Develops and updates an appropriate plan of care in collaboration with patient/family.
• Implements individualized treatment plans using evidence-based techniques.
• Performs reassessments and adjusts plans as needed.
• Communicates patient progress to supervisor and care team.
• Provides documentation per policies and regulatory requirements.
