Job summary

Tender Touch Rehab Services is seeking a licensed Physical Therapist for per diem work in a SNF/Long-Term Care setting in Philadelphia, PA, to provide high-quality rehab services.

You will evaluate patients with mobility, strength, balance and functional impairments; create and implement personalized care plans; collaborate with the care team; and ensure accurate documentation to meet regulatory requirements.

Qualifications

• Must be a licensed Physical Therapist to apply.
• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Current license as Physical Therapist in the state of practice.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
