OneWell Health Care is seeking a dedicated Per Diem Occupational Therapist (OTR/L) to provide client-centered therapy for individuals with IDD in home and community settings across Pennsylvania.

You will evaluate functional needs, develop goal-based plans, and collaborate with caregivers to enhance independence and quality of life. Flexible scheduling and field-based work characterize this role.

Qualifications

• Bachelor's or Master's degree in Occupational Therapy from an accredited program.
• Current and valid OTR/L licensure in the state of practice (PA).
• NBCOT certification and any state-specific certifications required.
• Strong clinical, documentation, organizational, and communication skills.
• Proficiency with EHR systems and assessment tools.

Responsibilities

• Conduct functional assessments and develop goal-based treatment plans.
• Provide home and community-based occupational therapy.
• Support daily living and adaptive skills through therapy.
• Recommend assistive devices and environmental modifications.
• Educate clients, families, and caregivers on strategies and home programs.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate clinical documentation and attend required trainings.
