Job summary

OneWell Health Care is seeking a compassionate Occupational Therapist (OTR/L) to provide personalized therapy to individuals with IDD in home and community settings across Pennsylvania. This per-diem, field-based role offers flexible scheduling and the chance to support independence and meaningful participation.

You will assess, create goal-based treatment plans, educate families, and collaborate with an interdisciplinary team.

Qualifications

• Must hold a degree in Occupational Therapy from an accredited program.
• Current OTR/L licensure in Pennsylvania.
• NBCOT certification and any state-specific certifications required.
• Strong clinical, documentation, organizational, and communication skills.

Responsibilities

• Conduct functional, standardized, and observational assessments.
• Develop and update individualized treatment plans.
• Provide evidence-based OT in home and community settings.
• Help individuals strengthen daily living, functional, and adaptive skills.
• Recommend adaptive equipment, assistive devices, and environmental modifications.
• Educate individuals, families, and caregivers on therapeutic strategies.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
