Job summary

OneWell Health Care is seeking a dedicated Per Diem Occupational Therapist (OTR/L) to provide personalized therapy in home and community-based settings in Pennsylvania.

You will evaluate functional needs, develop goal-based treatment plans, and collaborate with caregivers and other professionals to support independence, meaningful participation, and quality of life. Flexible scheduling and field-based travel are supported.

Qualifications

• Bachelor’s or Master’s degree in Occupational Therapy.
• Current and valid OTR/L licensure in PA.
• NBCOT certification and state-specific certifications as required.

Responsibilities

• Conduct functional, standardized, and observational assessments.
• Develop and update individualized treatment plans.
• Provide evidence-based occupational therapy in home and community-based settings.
• Help individuals strengthen daily living, functional, and adaptive skills.
• Recommend adaptive equipment and environmental modifications.
• Educate individuals, families, and caregivers on therapeutic strategies.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
