OneWell Health Care seeks a compassionate Occupational Therapist (OTR/L) to provide home and community-based therapy in Pennsylvania. The role is per diem in McKeesport, with flexible scheduling and field-based work to support IDD clients.

You will assess functional needs, develop goal-based plans, collaborate with families and interdisciplinary teams, and document care while traveling to service locations.

Qualifikationen

• Bachelor's or Master's degree in Occupational Therapy from an accredited program.
• Current and valid OTR/L licensure in the state of practice.
• NBCOT certification and any state-specific certifications required for practice.
• Strong clinical, documentation, organizational, and communication skills.
• Proficiency with OT assessment tools, functional evaluation methods, sensory integration interventions, assistive technology, adaptive equipment, and EHR systems.
• Valid driver's license with reliable transportation and willingness to travel to home/community locations.

• Conduct functional, standardized, and observational assessments.
• Develop and update individualized treatment plans.
• Provide evidence-based occupational therapy in home and community-based settings.
• Help individuals strengthen daily living, functional, and adaptive skills.
• Recommend adaptive equipment and environmental modifications.
• Educate individuals, families, and caregivers on therapeutic strategies.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
