Job summary

OneWell Health Care is seeking a compassionate Occupational Therapist (OTR/L) to provide personalized therapy in home and community-based settings throughout Pennsylvania. This per-diem role focuses on evaluating functional needs, creating goal-based plans, and collaborating with caregivers to improve independence.

Applicants must hold an OTR/L license in PA and NBCOT certification, with strong clinical, documentation, and communication skills.

Qualifications

• Bachelor's or Master's degree in Occupational Therapy from an accredited program.
• Current and valid OTR/L licensure in the state of practice.
• NBCOT certification.
• Strong clinical, documentation, organizational, and communication skills.
• Willingness to travel to home and community-based service locations.

Responsibilities

• Conduct functional, standardized, and observational assessments.
• Develop and update individualized treatment plans.
• Provide evidence-based occupational therapy in home and community-based settings.
• Help individuals strengthen daily living, functional, and adaptive skills.
• Recommend adaptive equipment, assistive devices, and environmental modifications.
• Educate individuals, families, and caregivers on therapeutic strategies.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
