Job summary

OneWell Health Care is seeking a dedicated Per Diem Occupational Therapist (OTR/L) to provide personalized therapy to individuals with IDD in home and community-based settings in Pennsylvania.

You will evaluate functional needs, develop goal-based treatment plans, and collaborate with caregivers and other professionals to support independence, meaningful participation, and quality of life.

Qualifications

• OTA/OTR licensed in Pennsylvania with NBCOT certification.
• Degree from an accredited Occupational Therapy program.
• Experience in home health or community settings preferred.

Responsibilities

• Conduct functional and observational assessments.
• Develop and update individualized treatment plans.
• Deliver evidence-based occupational therapy at home and in the community.
• Help individuals improve daily living and adaptive skills.
• Recommend assistive devices and environmental mods.
• Educate clients, families, and caregivers on strategies.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate clinical documentation in a timely manner.
• Participate in meetings and trainings as required.
