OneWell Health Care seeks a dedicated Per Diem Occupational Therapist (OTR/L) to provide personalized therapy to individuals with IDD in home and community settings across Pennsylvania. Evaluate functional needs, develop and update treatment plans, and collaborate with caregivers to support independence and quality of life.

Qualifications

• Bachelor's or Master's degree in Occupational Therapy from an accredited program.
• Current and valid OTR/L licensure in the state of practice.
• Current NBCOT certification and any state-specific certifications required.

Responsibilities

• Evaluate functional need and develop goal-based treatment plans.
• Provide evidence-based OT in home and community-based settings.
• Educate individuals, families and caregivers on therapies.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate clinical documentation.
• Travel to home and community-based service locations.
