Job summary

OneWell Health Care is seeking a dedicated Per Diem Occupational Therapist (OTR/L) to provide personalized therapy to individuals with IDD in home and community-based settings in Pennsylvania. You will evaluate needs, develop goal-based plans, and collaborate with caregivers to promote independence and quality of life.

Qualifications

• Bachelor's or Master's degree in Occupational Therapy required.
• Current and valid OT registration/licensure (OTR/L) in the state of practice.
• NBCOT certification and any required state certifications.
• Strong clinical, documentation, organizational, and communication skills.
• Proficiency with OT assessment tools, functional evaluation methods, sensory integration, and assistive tech; experience with EHR systems.
• Valid driver's license and willingness to travel to home and community sites.
• English proficiency required; bilingual (Spanish) a plus.

Responsibilities

• Conduct functional, standardized, and observational assessments.
• Develop and update individualized treatment plans.
• Provide evidence-based OT in home and community settings.
• Strengthen daily living, functional, and adaptive skills.
• Recommend adaptive equipment and environmental modifications.
• Educate individuals, families, and caregivers on strategies.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
