Job summary

Omega Home Health Care Services Inc. is seeking a compassionate Occupational Therapist to join our home health team on a per diem basis in California.

You will evaluate patients in their homes, develop individualized treatment plans, and deliver therapy per physician orders. Responsibilities include instructing patients and caregivers, monitoring progress, collaborating with a multidisciplinary team, and maintaining compliant documentation.

Qualifications

• Licensed OT in California to practice in home health.
• Graduation from an accredited Occupational Therapy program.
• Minimum 1 year of home health experience preferred.
• Valid driver's license and reliable transportation.
• Strong interpersonal, communication, and organizational skills.
• EMR proficiency is a plus.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans
• Deliver occupational therapy services in patients' homes in accordance with physician orders and company protocols
• Instruct patients and caregivers in use of adaptive equipment and techniques to improve daily living skills
• Monitor patient progress and modify treatment plans as needed
• Collaborate with multidisciplinary team including nurses, physical therapists, and social workers
• Maintain accurate and timely documentation in compliance with California state and federal regulations
• Ensure patient safety and adherence to infection control procedures in home environments
