Job summary

Omega Home Health Care Services, Inc. seeks a compassionate Occupational Therapist to join our California home health team on a per diem basis. You will evaluate patients, create and implement individualized treatment plans, and educate patients and caregivers on adaptive techniques to improve daily living.

Travel within the region and strict documentation are required. Ideal candidates hold a California OT license, have at least 1 year of home health experience, and possess strong communication

Qualifications

• Licensed to practice Occupational Therapy in California.
• Graduate of an accredited Occupational Therapy program.
• Minimum 1 year of home health or similar setting experience preferred.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Deliver occupational therapy services in patients’ homes per physician orders and company protocols.
• Instruct patients and caregivers on adaptive equipment and daily living techniques.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with nurses, physical therapists, and social workers.
• Maintain accurate documentation compliant with California state and federal regulations.
• Ensure patient safety and infection control in home environments.
