Job summary

Omega Home Health Care Services Inc. seeks a compassionate Occupational Therapist to join our home health team on a per diem basis.

The OT will provide rehabilitative services in patients' homes to help regain independence, under California licensure and in line with physician orders. The role requires travel within a designated region, per visit pay, and adherence to infection control and documentation standards.

Qualifications

• Must hold a current and valid California OT license.
• Graduate of an accredited OT program to practice.
• Minimum of 1 year experience in home health or related setting preferred.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Deliver occupational therapy services in patients' homes per physician orders and company protocols.
• Instruct patients and caregivers in use of adaptive equipment and techniques to improve daily living skills.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with multidisciplinary team including nurses, physical therapists, and social workers.
• Maintain accurate and timely documentation in compliance with California regulations.
• Ensure patient safety and infection control in home environments.
