Job summary

Omega Home Health Care Services, Inc. is seeking a compassionate Occupational Therapist to join our home health team on a per diem basis. You will provide rehabilitative services to patients in their homes to help them regain independence and improve quality of life.

Candidates must be licensed in California and experienced in home health or related settings. The role involves travel within a designated region and is paid by the visit, with flexible scheduling.

Qualifications

• Licensed Occupational Therapist in California.
• Graduate of an accredited OT program.
• Minimum 1 year of home health experience preferred.
• Valid driver’s license and reliable transportation.
• Strong interpersonal and communication skills.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans
• Deliver occupational therapy services in patients’ homes per physician orders and company protocols
• Instruct patients and caregivers in use of adaptive equipment and techniques to improve daily living skills
• Monitor patient progress and modify treatment plans as needed
• Collaborate with multidisciplinary team including nurses, physical therapists, and social workers
• Maintain accurate and timely documentation in compliance with California state and federal regulations
• Ensure patient safety and adherence to infection control procedures in home environments
