Job summary

Qualifications

• Currently licensed Occupational Therapist in California.
• Minimum 1 year home health or similar setting experience.
• Valid driver’s license and reliable transportation.
• Excellent communication, interpersonal, and organizational skills.
• Proficiency with EMR systems is a plus.

Responsibilities

• Perform comprehensive patient evaluations and create individualized treatment plans.
• Deliver OT services in patients' homes per physician orders and protocols.
• Instruct patients and caregivers in adaptive equipment use and daily living techniques.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with nurses, PTs, social workers and other team members.
• Maintain accurate documentation in line with regulations.
• Ensure safety and infection control in home environments.
