Omega Home Health Care Services Inc. seeks a compassionate Occupational Therapist to join our home health team on a per diem basis. Provide rehabilitative therapies in patients' homes, guided by physician orders and company protocols, with travel within a designated region.

Licensed OT in California with a relevant program background, plus at least 1 year in home health preferred. Strong communication, organizational skills, and reliable transportation required.

Qualifications

• Current and valid California OT license.
• Graduate of an accredited Occupational Therapy program.
• Minimum of 1 year experience in a home health or similar setting preferred.
• Valid driver’s license and reliable transportation.
• Strong interpersonal, communication, and organizational skills.
• Proficiency with electronic medical records (EMR) systems is a plus.

Responsabilités

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Deliver occupational therapy services in patients’ homes in accordance with physician orders and company protocols.
• Instruct patients and caregivers in use of adaptive equipment and techniques to improve daily living skills.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with multidisciplinary team including nurses, physical therapists, and social workers.
• Maintain accurate and timely documentation in compliance with California state and federal regulations.
• Ensure patient safety and adherence to infection control procedures in home environments.
