Job summary

ComplexCare Solutions is seeking a per diem Nurse Practitioner to provide in-home and virtual assessments in Jackson, CA. This role focuses on enhancing clinical outcomes through thorough health evaluations and care management for patients.

Ideal candidates will have an active nursing license and ANCC or AANP certification, with strong communication and customer service skills. Compensation is performance-based, with rates ranging up to $10,000 monthly depending on assessments completed.

Qualifications

• Prior experience in Home Health is a plus.
• Understanding Medicare, Medicaid, and Health Plan structures is beneficial.
• Ability to practice autonomously in a remote clinical environment.

Responsibilities

• Perform comprehensive assessments and manage care plans.
• Document clinical findings according to compliance standards.
• Educate members on chronic condition management.

Communication with elderly and chronically ill patients

Education
