Job summary

MJHS Hospice and Palliative Care seeks a Nurse Practitioner to provide palliative care consultations in nursing homes, hospitals, and community settings. The NP will develop care plans, perform examinations, and collaborate with an interdisciplinary team to ease patient and family burden.

Requirements include a Master's in Nursing, NY RN licensure with NP certification, and preferred hospice/palliative or geriatric NP experience. Travel within the assigned area is required; CPR-BLS is preferred.

Qualifications

Master's degree in Nursing and graduation from an approved Nurse Practitioner program.
New York State RN licensure and NP certification required.
Hospice, Palliative Care and/or Geriatric NP experience preferred.
1 year of clinical NP experience preferred.
CPR-BLS preferred.

Responsibilities

Staff palliative care consultation services in nursing homes, hospitals or community settings.
Develop and advance care plans and provide treatment recommendations.
Communicate with referral sources and document encounters accurately.
Travel within the assigned geographic area to provide patient care.
Collaborate with an interdisciplinary team and educate staff as needed.
