Job summary

Qualifications

• 1+ year clinical experience as an NP provider preferred.
• Hospice, Palliative Care, and/or Geriatric nursing experience preferred.
• CPR-BLS and valid NYS Driver's license preferred.

Responsibilities

• Implement specialist palliative care consultation services.
• Maintain effective communication with referral sources.
• Perform timely, accurate and compliant documentation.
