Job summary

MJHS seeks a Nurse Practitioner provider to staff palliative care consultation services in diverse settings, including nursing homes, hospitals, and community environments. The NP will assess patients, develop care plans, and collaborate with an interdisciplinary team to relieve illness burden and improve quality of life.

Initial encounters guide ongoing visits; responsibilities include documentation, education, and on-call rotation support.

Qualifications

• Master's degree required; Nurse Practitioner program graduation.
• New York State RN licensure required.
• Hospice/Palliative Care and/or Geriatric NP experience preferred.
• Ability to travel within the assigned geographic area.
• Strong communication, listening and organizational skills.
• Ability to work within an integrated health care team.

Responsibilities

• Provide hospice and palliative care consultation in nursing homes, hospitals or community settings.
• Develop and implement care plans with an interdisciplinary team.
• Perform initial patient encounters and follow-up visits as needed.
• Ensure timely, accurate and compliant documentation.
• Support hospice operations and educational initiatives.
• Coordinate resources and personnel for comprehensive care.
