Job summary

MJHS Hospice and Palliative Care seeks a Nurse Practitioner to join our team providing palliative care consultations in nursing homes, hospitals, and community settings. You will manage symptoms, discuss goals of care, and develop comprehensive care plans with an interdisciplinary team.

Travel within the NYC metro area is required. Applicants should hold a Master's in Nursing and be a Graduate of an approved NP program, with NYS RN licensure and NP certification.

Qualifications

• Master's degree in Nursing and NP program completion required.
• New York State RN licensure + NP certification required.
• CPR-BLS certification required or preferred.
• Hospice & Palliative Care certification encouraged by year 2 of hire.
• Ability to travel within the assigned geographic area to provide patient care.

Responsibilities

• Staff palliative care consultation services within MJHS Hospice and Palliative Care.
• Provide specialized symptom management and goal setting discussions.
• Develop care plans and document encounters accordingly.
• Educate staff and support hospice operations including on-call rotation.
• Collaborate with an interdisciplinary team to coordinate patient care.
• May work in nursing homes, hospitals, or community settings.
