Job summary

MJHS Hospice and Palliative Care seeks a Nurse Practitioner (NP) to join a multidisciplinary team. You will provide palliative care consultations across nursing homes, hospitals, and community settings, aiming to alleviate symptom burden and improve quality of life for patients and families.

The NP works within an established care plan, collaborates with MDs and NPs, and participates in on-call rotations. Travel within the NY metro area is required.

Qualifications

Master's degree in Nursing and NP program completion.
New York State NP licensure required.
RN licensure in NY required.
CPR-BLS and valid NYS driver’s license preferred.
Hospice & Palliative Care certification encouraged by year 2.
Experience with hospice or palliative care settings is preferred.

Responsibilities

Staff palliative care consultations in nursing homes, hospitals, or community settings.
Develop and implement care plans to prevent or mitigate illness burden.
Educate staff and support hospice operations through involvement in interdisciplinary teams.
Participate in on-call rotations and face-to-face encounters as needed.
Travel within the assigned geographic area to provide patient care.
