Job summary

MJHS Hospice and Palliative Care is seeking a Nurse Practitioner (NP) provider to staff palliative care consultation services in nursing homes, hospitals or community settings. The NP will work with MDs and NPs to manage symptom relief, goal setting discussions, and hospice eligibility reviews.

The role requires an NP with strong communication, teaching and interdisciplinary teamwork skills, plus ability to travel within the NYC area to support patient care and care coordination.

Qualifications

• Master's degree in Nursing required.
• New York State RN licensure required.
• Hospice, Palliative Care and/or Geriatric NP experience preferred.

Responsibilities

• Respond to consultation requests with initial patient encounters.
• Develop and update care plans to prevent or mitigate illness burden.
• Conduct objective assessments and documentation of care.
• Educate staff and support hospice operations.
• Travel within assigned geographic area to provide patient care; participate in on-call rotation.
