Job summary

MJHS is seeking a Nurse Practitioner to join its Hospice and Palliative Care team. The NP Provider will conduct initial assessments, manage symptom control, and coordinate care plans in nursing homes, hospitals or community settings across Manhattan and Brooklyn.

The role requires a Master's in Nursing, NP certification, NYS RN licensure, and strong communication with patients, families and interdisciplinary teams.

Qualifications

• Master's degree in Nursing and NP program graduate required.
• New York State RN licensure required.
• Hospice/Palliative Care certification strongly encouraged by year 2 of hire.
• CPR-BLS certification preferred.

Responsibilities

• Provide palliative care consultations and follow-up visits.
• Collaborate with an interdisciplinary care team.
• Travel within the assigned geographic area to provide patient care.
• Participate in on-call rotation and after-hours coverage as needed.
• Maintain accurate documentation and care plans.
