Job summary
Community Health Resources (CHR) is seeking an Licensed Practical Nurse for Pathways OTP in Enfield, CT. This per diem on-site role requires Saturday shifts from 5:30 AM to 11:30 AM, with weekday availability as needed.
The LPN will support methadone maintenance, contribute to care coordination, and work within a multidisciplinary team. The position emphasizes quality treatment delivery, documentation, and adherence to MAT standards.
Qualifications
- Nursing degree or related health care field preferred.
- No prior work experience required.
- Must hold a Connecticut nursing license (RN or LPN) with CPR certification and a valid driver’s license.
Responsibilities
- Perform initial nursing assessments and document ongoing assessments of patients’ response to treatment.
- Develop, implement and evaluate nursing care plans according to age specific needs of patients.
- Record progress in a timely manner and ensure accurate input of computer data.
- Execute the medical and medication regimen of care as prescribed by the physician.
- Provide patients with daily methadone doses and prescribed take-home doses.
- Monitor urine toxicology screens and breathalyzers; administer PPD tests.
