Job summary

Community Health Resources seeks a Licensed Practical Nurse to join the Pathways OTP in Enfield, CT. This per diem role covers Saturdays 5:30AM-11:30AM with potential weekday coverage as needed, supporting a methadone maintenance program.

You will perform nursing assessments, develop care plans, administer methadone, and coordinate care with counselors and physicians. Connecticut LPN license and CPR certification are required.

Qualifications

• Education: Degree or certification in nursing or a related health care field.
• Experience: none required.
• Licensure/Certification/Registration: Current CT LPN license and CPR certification; valid driver’s license.

Responsibilities

• Perform initial nursing assessments and document patients’ responses to treatment.
• Develop, implement and evaluate nursing care plans per patient age-specific needs; record progress.
• Input data accurately into the computer system and coordinate with medical staff.
• Administer prescribed methadone regimen and take-home doses as directed by the physician.
• Monitor urine toxicology, breathalyzers, and respond to medical/psychiatric emergencies per CHR policies.

ABOUT THE PROGRAM

Pathways Opiate Treatment Program

Pathways is an accredited opiate Treatment Program that provides MAT (exclusively methadone), individual therapy group counseling, psycho-educational groups, individualized treatment planning, recovery coaching, harm reduction services, and ongoing assessment and referrals. Pathways runs with a supportive multidisciplinary team model including a great blend of clinical and medical staff. At Pathways, the team and program work hard to save lives and reduce risks every day.

ABOUT THE POSITION:

To be a part of a team responsible for the operation of a methadone maintenance program.

By:

• Providing quality treatment while working with the team to provide support to clients with addictions and/or co-occurring disorders.
• Participating in case reviews, planning, and coordination of care.

DUTIES & RESPONSIBILITIES:

• Perform initial nursing assessments and document ongoing assessments of patients’ response to treatment.
• Develop, implement and evaluate nursing care plans according to age specific needs of patients. Record progress in a timely manner.
• Responsible for accurate input of computer data.
• Execute the medical and medication regimen of care as prescribed by the physician.
• Provides clients with daily methadone doses and prescribed take-home doses.
• Monitor urine toxicology screens and breathalyzers.
• Administer PPD tests.
• Educate the patient/family toward an understanding of healthy living.
• Respond to medical/psychiatric emergency needs of patients, following CHR policies and procedures.
• Adhere to the standards for Medication Assisted Treatment as maintained by DPH, DMHAS, Joint Commision, D.E.A., and the U.S. Federal Government.
• Monitoring of methadone stock and ordering as described in CHR policies and all regulatory bodies.
• Restocking methadone supply upon delivery.
• Communicate with counselors and Medical Doctor regarding patient status and progress.
• Maintains documentation in compliance with agency and program standards.
• Participates in staff meetings, supervision, trainings and other meetings as assigned.
• Additional duties as required.

QUALIFICATIONS:

Education: Degree/Certification in nursing or in a related health care field preferred.

Experience: none required

Licensure/Certification/Registration: Current RN or LPN license in the State of Connecticut required, Certification in CPR, and must have a valid driver’s license.

COMPENSATION*:

$30/hour

• Our per diem and fee for service positions offer a flat hourly rate.
