Job summary

Community Health Resources, Inc. is seeking a Licensed Practical Nurse for Pathways OTP in Enfield, CT. This per diem, Saturday 5:30AM-11:30AM position is on-site with duties including nursing assessments, care planning, medication administration, and coordination of care within the methadone maintenance program.

Requires current CT LPN license and CPR certification. Per diem role offers $30/hour with potential for additional duties as needed.

Qualifications

• Education: Degree/Certification in nursing or in a related health care field preferred.
• Experience: none required.
• Licensure/Certification/Registration: Current RN or LPN license in the State of Connecticut required, CPR certification.

Responsibilities

• Perform initial nursing assessments and document ongoing assessments of patients’ response to treatment.
• Develop, implement and evaluate nursing care plans according to age specific needs of patients.
• Responsible for accurate input of computer data.
• Execute the medical and medication regimen of care as prescribed by the physician.
• Provide patients with daily methadone doses and prescribed take-home doses.
• Monitor urine toxicology screens and breathalyzers.
• Educate the patient/family toward an understanding of healthy living.
• Respond to medical/psychiatric emergency needs of patients, following CHR policies and procedures.
• Adhere to standards for Medication Assisted Treatment as maintained by regulatory bodies.

JOB TITLE: Licensed Practical Nurse

SCHEDULED HOURS: Saturdays 5:30AM-11:30AM required/weekday availability as needed

PROGRAM/LOCATION:Pathways OTP, Enfield, CT

ABOUT THE PROGRAM : Pathways Opiate Treatment Program

Pathways is an accredited opiate Treatment Program that provides MAT (exclusively methadone), individual therapy group counseling, psycho-educational groups, individualized treatment planning, recovery coaching, harm reduction services, and ongoing assessment and referrals. Pathways runs with a supportive multidisciplinary team model including a great blend of clinical and medical staff. At Pathways, the team and program work hard to save lives and reduce risks every day.

ABOUT THE POSITION:

To be a part of a team responsible for the operation of a methadone maintenance program.

By:

• Providing quality treatment while working with the team to provide support to clients with addictions and/or co-occurring disorders.
• Participating in case reviews, planning, and coordination of care.

DUTIES & RESPONSIBILITIES :

• Perform initial nursing assessments and document ongoing assessments of patients’ response to treatment.
• Develop, implement and evaluate nursing care plans according to age specific needs of patients. Record progress in a timely manner.
• Responsible for accurate input of computer data.
• Execute the medical and medication regimen of care as prescribed by the physician.
• Provides clients with daily methadone doses and prescribed take-home doses.
• Monitor urine toxicology screens and breathalyzers.
• Administer PPD tests.
• Educate the patient/family toward an understanding of healthy living.
• Respond to medical/psychiatric emergency needs of patients, following CHR policies and procedures.
• Adhere to the standards for Medication Assisted Treatment as maintained by DPH, DMHAS, Joint Commision, D.E.A., and the U.S. Federal Government.
• Monitoring of methadone stock and ordering as described in CHR policies and all regulatory bodies.
• Restocking methadone supply upon delivery.
• Communicate with counselors and Medical Doctor regarding patient status and progress.
• Maintains documentation in compliance with agency and program standards.
• Participates in staff meetings, supervision, trainings and other meetings as assigned.
• Additional duties as required.

QUALIFICATIONS :

Education: Degree/Certification in nursing or in a related health care field preferred.

Experience: none required

Licensure/Certification/Registration: Current RN or LPN license in the State of Connecticut required, Certification in CPR, and must have a valid driver’s license.

COMPENSATION*:

$30/hour

*Our per diem and fee for service positions offer a flat hourly rate.
