Job summary

All American Assisted Living at Raynham is seeking an LPN to provide resident-centered nursing support in a compassionate, team-based environment. You will administer medications, monitor health, and coordinate services under nursing leadership to promote residents’ independence, dignity, and quality of life.

The role demands adherence to Massachusetts LPN scope, strong observation skills, and excellent documentation.

Qualifications

• Must hold an active Massachusetts LPN license.
• Knowledge of practical nursing standards and medication administration.
• Strong observation and resident-monitoring skills.
• Excellent documentation and communication with residents, families, providers, and pharmacies.
• Ability to work within the LPN scope of practice and escalate when required.
• Ability to supervise and provide guidance to resident-care staff.
• Strong English written and verbal communication.

Responsibilities

• Administer medications and treatments according to orders and procedures.
• Monitor residents for changes in health, cognition, mood, and mobility.
• Provide direct nursing care within MA LPN scope of practice.
• Collect and document health information for residents.
• Participate in assessments and care-plan reviews.
• Communicate with Wellness team and leadership about care needs.
• Support admissions, transitions, and changes in level of care.
