Job summary

All American Assisted Living at Raynham is seeking an LPN for our Wellness team in a part-time evening role. You will provide resident-centered nursing support, administer medications per orders, and monitor health under the guidance of nursing leadership.

The ideal candidate holds a valid MA LPN license and has strong documentation, communication, and teamwork skills to support residents’ health and dignity.

Qualifications

• Massachusetts LPN license in good standing.
• Knowledge of practical nursing standards and medication administration.
• Strong observation and resident-monitoring skills.
• Ability to document and report changes clearly.
• English communication skills, spoken and written.

Responsibilities

• Administer medications and treatments per orders and procedures.
• Monitor residents for changes in condition and daily functioning.
• Provide direct nursing care within the LPN scope of practice.
• Document resident health information and assessments.
• Coordinate care plans and communicate with families and providers.
• Respond promptly to emergencies and escalate when required.
• Support admissions and transitions between levels of care.
